The following hard-copy forms MUST be submitted at registration. Teams that fail to submit these forms will not be permitted to compete until the forms have been received
- Registration Form (one per association)
- Banquet Registration Form (one per association)
- Eligibility Form (one per school)
- Practice Eligibility Form (one per school; note: practice is not needed for March Break week and during exams)
- Supervision Form (one per school)
- Release of Liability Waiver (one per athlete)
- OFSAA Rules of Behaviour (one per school)
* OFSAA photo policy is now within the Rules of Behaviour form
