PRIOR TO THE EVENT:
1) submit a completed AELS form by 11:59 pm on November 20, 2019
2) take part in the coaches meeting on November 21, 2019 at 8 pm via video-conference. The link for this meeting will be sent out Monday, November 18. ALL SCHOOLS MUST HAVE A TEACHER-COACH REPRESENTATIVE attend this meeting.
3) upload all forms to website by 3:00 pm on Friday, November 22, 2019. Make sure that all forms are complete and signed properly.
- AELS with all required signatures
- OFSAA Rules of Behaviour form
- OFSAA Liability Form (1 per participant)
- Concussion Review Confirmation Form (1 per participant)
- Team roster form
1) present cheque for OFSAA participation fee ($3- x # of student - athletes) - payable to OFSAA
2) pesent cheque for $250- for entry fee - payable to H.S.S.A.A. c/o Darrin Curtis
