Registration Forms and Fees
1. Online OFSAA HUB Registration – DUE TUESDAY, FEBRUARY 20 AT 11:59PM
Athletic Directors must start the registration process by completing & uploading the following mandatory forms through the OFSAA HUB:
2. Remaining Mandatory Forms – DUE FRIDAY, FEBRUARY 23 AT 3:30PM
Athletic Directors/Coaches must submit and upload the remaining mandatory forms with appropriate signatures:
- Student – Waiver of understanding (One form per student. All forms should be scanned into one file)
- Osler Bluff Waiver (Group Name: OFSAA, Midweek Number: OFSAA) One form per person skiing (does not need to be uploaded – just completed online)
- Coach – Confirmation and Release (One form per coach. All forms should be scanned into one file)
- OFSAA Supervision Form
- OSBIE SKI/SNOWBOARDING COMPETITIONS RELEASE (Applicable forms below)

- Student-Athlete/Parent Concussion Review Confirmation– Confirmation de l’examen de la commotion d’athlète/parent
- Participation Informed Consent and Assumption of Risk (Under 18)
- Photo Release Form
- Adult Release of Liability (18+) (If no one is over the age of 18 and you do not need to distribute)

ROWANS LAW:
As mandated by Rowan’s Law, every student-athlete, coach and trainer is required to confirm they have reviewed the Concussion Awareness Resource and the OFSAA Concussion Code of Conduct. In order to participate in this event it is mandatory that a confirmation form for each participant be completed and submitted with registration.

Championship Fees :
Submit on Sunday, February 25th from 1:30pm – 4:00pm @ the OFSAA Alpine Championship Registration Desk, Osler Bluff Ski Club
- Each school (one coach) must bring five (5) cheques to Registration — one for the Bib Deposit, one for the total of all OFSAA Participation fees (Association), one for the total of all Osler Alpine Race Entry fees, one for the total of the OFSAA Athlete Entry fee, and one for the total of all Banquet fees.
