PRIOR TO THE EVENT:
1) Complete the Football Team and Season Form by xxxxxxxxx.
2) Upload a completed OFSAA HUB Roster/Eligibility form by xxxxxxxx . This form is to be completed by the Athletic Director at each participating school. Please ensure that you fill in all of the columns, including POS, #, ALT #, HEIGHT, & GRADE. This information is important to the convenor.
Also please submit a numeric roster for web page – in word format – with number, name, position
Add logo to top and coaches on bottom
This needs to be emailed to [email protected]
3) Mandatory Form – TO BE UPLOADED ON THE HUB – DUE xxxxxxx AT 11:59PM
Athletic Directors must start the registration process by completing & uploading the following mandatory forms through the OFSAA HUB:
- Printed & Signed HUB Eligibility Roster (example)
Note: As stated previously, only 65 participants per team (56 players and 9 coaches) can register. Only these 65 individuals should appear on the eligibility sheet (you may cross off names by hand if needed).
4) Athletic Directors/Coaches must submit and upload the remaining mandatory forms with appropriate signatures by 3:30 pm on xxxxxxxx:
- Student – Waiver of understanding (One form per student. All forms should be scanned into one file)
- Coach – Confirmation and Release (One form per coach. All forms should be scanned into one file)

FORMS TO BE DISTRIBUTED TO STUDENT-ATHLETES (IT IS RECOMMENDED THAT THE COMPLETED FORMS ARE COLLECTED BY COACHES):
- OFSAA Rules of Behaviour
- Student-Athlete/Parent Concussion Review Confirmation– Confirmation de l’examen de la commotion d’athlète/parent
- Participation Informed Consent and Assumption of Risk (Under 18)
- Photo Release Form
- Adult Release of Liability (18+) (If no one is over the age of 18 and you do not need to distribute)
FORMS TO BE REVIEWED BY COACHES:
- Coach/Trainer Concussion Review Confirmation – Confirmation de l’examen des commotions cérébrales entraîneurs/soigneurs
5) Take part in the coaches meeting on xxxxxxxxxxx. via video-conference. This meeting is MANDATORY for all teacher-coach representatives of the qualifying teams. This meeting will take place via ZOOM and the link for this meeting will be sent out by. xxxxxxx.
GAME DAY OPERATIONS: Upon your arrival please ensure that you:
1) follow all local Public Health, School Board and OFSAA Protocols for entry and participation in the event.
2) respect the game day protocols; specific details regarding game day protocols will be made available as we get closer to the event and these details will also be shared at the Coaches Meeting on xxxxxxxx.
3) present a cheque (payable to OFSAA) in the amount of $147- for the OFSAA participation fees. This fee has been standardized for all teams who participate in the OFSAA FOOTBALL FESTIVAL
4) THERE WILL BE NO ENTRY FEE charged to each team for this year.
