ENTRY DEADLINE: 

REMINDER: ONLY 7 PARTICIPANTS PER TEAM (COMBINATION OF PLAYERS AND TEAM PERSONNEL) CAN REGISTER.

ONLY REGISTERED PARTICIPANTS MAY REPRESENT THE SCHOOL AND/OR ATTEND CHAMPIONSHIP ACTIVITIES. 

Championship Registration Steps:

2.  Download, Sign, Scan and Upload Mandatory Registration Forms
3.  On-Site Fee Payment

1. Online OFSAA HUB Registration – DUE THURSDAY, MARCH 7 AT 11:59PM

 Athletic Directors must start the registration process by completing & uploading the following mandatory forms through the OFSAA HUB:

  • Printed & Signed HUB Eligibility Roster (example) 
    Note: As stated previously, only 7 participants per team (combination of players and team personnel) can register. Only these 7 individuals should appear on the eligibility sheet (you may cross off names by hand if needed).
  • Team & Season Information Form
    Note: Please complete this form with as much detail as possible. This greatly assists in the seeding process.
2. Remaining Mandatory Forms – DUE SUNDAY, MARCH 17 AT 5:00PM

Athletic Directors/Coaches must submit and upload the remaining mandatory forms with appropriate signatures:

As mandated by Rowan’s Law, every student-athlete, coach and trainer is required to confirm they have reviewed the Concussion Awareness Resource and the OFSAA Concussion Code of Conduct.  In order to participate in this event it is mandatory that a confirmation form for each participant be completed and submitted with registration.

On-site registration and team photos for all teams will occur at Cataraqui Golf & Country Club on March 19 between 10am -2pm. Please book time online for your registration/photos (1 time per team) at the following link: OFFSA 2024 Registration and Photo Booking

Food Bank Donation – “Stuff the Volvo” – Please bring a non-perishable food item to fill the Van Herpt Volvo in support of the Partners in Mission Foodbank. For a list of most needed items please click here.

Championship Fees:

Please bring three (3) separate cheques (team entry fee, banquet fee, OFSAA participation fee).
