REMINDER: ONLY 7 PARTICIPANTS PER TEAM (COMBINATION OF PLAYERS AND TEAM PERSONNEL) CAN REGISTER.

ONLY REGISTERED PARTICIPANTS MAY REPRESENT THE SCHOOL AND/OR ATTEND CHAMPIONSHIP ACTIVITIES.

4.  On-Site Fee Payment

1. Athletic Director or designate completes, scans, and submits all Championship OFSAA HUB forms listed below

UPDATE & PRINT ROSTER
Teacher-coaches ensure all student-athletes are listed in the OFSAA HUB on an appropriate Eligibility Roster. Each of these rosters needs to be printed and signed.DOWNLOAD & PRINT FORMS
Teacher-coaches download, print, and complete the following additional forms for all rostered athletes and teacher-coaches. These forms are also available for download from the OFSAA HUB.

  • Download and print the mandatory forms (below)
  • Obtain appropriate signatures
  • Scan and save file
Note: Forms must be uploaded using the filename examples in parenthesis.  All concussion confirmation forms must be scanned into one file.

New for 2020!  As mandated by Rowan’s Law, every student-athlete, coach and trainer is required to confirm they have reviewed the Concussion Awareness Resource and the OFSAA Concussion Code of Conduct.  Mandatory receipt of forms with appropriate signatures confirming the resource and code have been reviewed is required in order to participate at OFSAA.  Student-athletes under the age of 18 are required to have a parent or legal guardian sign the confirmation as well.

3. SCAN & UPLOAD FORMS
Athletic Directors or designate scan and upload completed forms to the OFSAA HUB no later than March 9 @ 11:59pm

4.  On-Site Registration: Please bring three (3) separate cheques (team entry fee, banquet fee, OFSAA participation fee).
