Association Convenor Information

Please complete the following to confirm you are the Association Convenor. This form can only be filled out ONCE per Association. Therefore, if you are NOT the Association Convenor, you should not be filling out this form. If you are a teacher-coach or athletic director looking to register a student-athlete(s) who qualified, you should register them through the OFSAA HUB. The Association Convenor will update their qualifying information in this form.
