Association Commitment Form

Associations withdrawing entry commitments after the September 30th deadline will be subject to penalties outlined in the OFSAA Constitution.
Please make sure that your member schools are aware of the possible ramifications of withdrawal from a championship BEFORE submitting this commitment form to the OFSAA. The deadline for all fall sport commitments is September 30th, 2024.
